Your choices

For certain information, you may tell us your preferences about sharing with family, friends, or others involved in your care; disaster-relief activities; fundraising; and other uses described by law. Uses not otherwise permitted—including most marketing, sale of health information, and many uses of psychotherapy or substance-use-disorder treatment records—require written authorization when applicable. You may revoke an authorization in writing, except to the extent already acted upon.

How we may use and share information

We may use or share health information for treatment, payment, and healthcare operations; public-health and safety activities; reporting suspected abuse or neglect; health oversight; workers’ compensation; responding to lawsuits or lawful requests; working with medical examiners or funeral directors; organ and tissue donation; research when legally permitted; and other uses required or allowed by law. Additional protections apply to certain records, including substance-use-disorder treatment information.

Our responsibilities

We are required by law to maintain the privacy and security of protected health information, notify affected individuals following a breach when required, follow the duties and privacy practices described in the current notice, and provide a copy upon request. We will not use or share information other than as permitted by law or authorized by you in writing.

Complaints and questions

Contact the privacy contact above with questions or complaints. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights through its complaint portal. We will not retaliate for filing a complaint.
